Sometimes a joint under strain may “pop out” of position and back in again (subluxation). This will be painful, but does not require reduction (relocation or returning to normal position). A subluxation is treated as a sprain injury. Dislocations are most common in the shoulders, elbows, fingers, and kneecap (patella). The reduction of a dislocation (returning the bone to its normal position) by non-physicians is controversial in wilderness medicine. There are many good reasons for reducing a dislocation in the backcountry, especially when rapid transport to a hospital is not possible: • Reduction is easier soon after injury, before swelling and muscle spasm develop.

58 Fractures Fractures Fractures A fracture is any break or crack in a bone. An open, or compound fracture occurs when the overlying skin at the fracture site has been punctured or cut. This can happen when a sharp bone end protrudes through the skin or from a direct blow which breaks the skin as it fractures the bone. The bone may or may not be visible in the wound. A closed fracture is one in which there is no wound on the skin anywhere near the fracture site. A closed fracture can become an open fracture if it is not handled carefully.

Symptoms include tenderness at the site, swelling, bruising, and pain with movement. Because these symptoms are also present with a fracture, it may be difficult to differentiate between the two. General Treatment First aid for sprains is primarily damage control and summarized by the acronym RICES: (Rest, Ice, Compression, Elevation and Stabilization). RICES should be maintained for the first 72 hours after any injury. Too often this is prematurely discontinued after only a few hours. Rest: Resting takes the stress off the injured joint and prevents further damage.
